A few additional points specific to PVA:
You may request a copy of your records at any time. We require you to provide government-issued photo identification to verify your identity, and must also complete OCA Form No. 960 - Authorization for Release of Health Information Pursuant to HIPAA prior to the release of your information.
We take your privacy seriously, and only authorize designated managers to process your request for medical records. This is done to limit the number of personnel who access or view your records, to better protect your privacy. Given the demands of emergency services response, an authorized manager may not be on-site at all times. Therefore, we ask that you contact us in advance to schedule an appointment, if you would like to receive a copy of your medical records at the PVA base. To schedule an appointment, or for assistance with this process, please call us at: 585-385-2401.
