Health Insurance Portability and Accountability Act (HIPAA)

Pittsford Volunteer Ambulance provides medical services for our patients, and as a healthcare provider organization we comply with all HIPAA and associated statutory requirements. PVA protects the privacy and security of health information, and provides our patients with certain rights with respect to their health information. This section outlines the HIPAA requirements PVA is in compliance with, including how your medical information may be accessed, used, and disclosed, and how you can get access to this information. Please review this carefully.

Pittsford Volunteer Ambulance is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide individuals with you with a notice of our legal duties and privacy practices with respect to PHI. PHI is information about you, including basic demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.

Notice of Privacy Practices

This Notice of Privacy Practices (Notice) describes how we may use and disclose PHI about you to carry out treatment, payment or health care operations and for other specified purposes that are permitted or required by law. The Notice also describes your rights with respect to PHI. Pittsford Volunteer Ambulance is required to follow the terms of this Notice. We will not use or disclose PHI about you without your written authorization, except as described in this Notice. We reserve the right to change our practices and this Notice and to make the new Notice effective for all PHI we maintain. Upon request, we will provide any revised Notice to you.

A few additional points specific to PVA:

You may request a copy of your records at any time. We require you to provide government-issued photo identification to verify your identity, and must also complete OCA Form No. 960 - Authorization for Release of Health Information Pursuant to HIPAA prior to the release of your information.

We take your privacy seriously, and only authorize designated managers to process your request for medical records. This is done to limit the number of personnel who access or view your records, to better protect your privacy. Given the demands of emergency services response, an authorized manager may not be on-site at all times. Therefore, we ask that you contact us in advance to schedule an appointment, if you would like to receive a copy of your medical records at the PVA base.
